Service Description

o Suggest and demonstrate techniques that can make essential activities possible or easier. o Collaborate with a home improvement contractor to develop a modification or build plan that will meet the needs of an individual or household. o. Work with clients after the modifications, equipment or specialized products have been installed in order to ensure safe usage. o Provide Caregiver Support o Establish a Senior Concierge o Recommend Community Resources based on individual need o Create a plan for support and independence with Activities of daily living o Implement a Wellness Plan o Create a support system o Provide Fall Prevention instruction o Provide Alzheimer’s & Dementia Support o Case Management
