Service Description

As a Respecting Choices Advance Care Facilitator, Jen helps both you and your family understand your goals and values. She will guide you with your Advance Directive, 5 wishes, and facilitate a discussion with you and your family to understand how you want your wishes carried out. Copies will be provided to distribute to necessary parties, family, medical doctors, etc. It's valuable to begin thinking and talking about future healthcare decisions if you had a sudden event, like a car accident or illness, and could not make your own decisions. This way, your choices will be well communicated and will provide comfort to your family knowing they are carrying out your wishes just as you desired. VIRTUAL OPTION: please email getinspiredbyjen@gmail.com to set up a virtual session if desired. Cost: $195.00 includes (2) 1 hour sessions
