As a Respecting Choices Advance Care Facilitator, Jen supports you and your family in comprehending your goals and values. She assists in navigating your Advance Directive, 5 wishes, and facilitates a discussion with you and your family to ensure a clear understanding of how you want your wishes executed. Copies will be provided for distribution to necessary parties, including family and medical professionals. Initiating discussions about future healthcare decisions becomes crucial, especially in unforeseen events like accidents or illnesses where you might be unable to make decisions. This proactive approach ensures your choices are well-communicated, offering comfort to your family as they carry out your wishes exactly as you intended.
