Stay Home: Aging in Place Evaluation

Service Description

This service prepares clients to age comfortably and safely in their home and considers the health and disability issues people face over a lifetime. This evaluation will include a home assessment that addresses individual routines, daily activities, and barriers that impact performing them. The evaluation includes: 1. Assessing your abilities, challenges and needs. 2. Providing results and recommended changes to increase safety and ease of use. 3. Identifying and recommending furnishings, equipment and techniques that can help with activities. (Optional) Ongoing Direct Occupational Therapy Services: see Aging in Place Services 1. Suggesting and demonstrating techniques that can make essential activities possible or easier. 2. Collaborating with a home improvement contractor to develop a modification or build plan that will meet the needs of an individual or household. 3. Working with clients after the modifications, equipment or specialized products have been installed in order to ensure safe usage.
